Background: Alcohol use is a key preventable risk factor for serious injury. To effectively prevent alcohol-related injuries, we rely on the accurate surveillance of alcohol involvement in injury events. This often involves the use of administrative data, such as International Statistical Classification of Diseases and Related Health Problems, Tenth Revision, Australian Modification (ICD-10-AM) coding. Objective: To evaluate the completeness and accuracy of using administrative coding for the surveillance of alcohol involvement in major trauma injury events by comparing patient blood alcohol concentration (BAC) with ICD-10-AM coding. Method: This retrospective cohort study examined 2918 injury patients aged ≥18 years who presented to a major trauma centre in Victoria, Australia, over a 2-year period, of which 78% ( n = 2286) had BAC data available. Results: While 15% of patients had a non-zero BAC, only 4% had an ICD-10-AM code suggesting acute alcohol involvement. The agreement between blood alcohol test results and ICD-10-AM coding of acute alcohol involvement was fair ( κ = 0.33, 95% confidence interval: 0.27–0.38). Of the 341 patients with a non-zero BAC, 82 (24.0%) had ICD-10-AM codes related to acute alcohol involvement. Supplementary factors Y90 Evidence of alcohol involvement determined by blood alcohol level codes, which specifically describe patient BAC, were assigned to just 29% of eligible patients with a non-zero BAC. Conclusion: ICD-10-AM coding underestimated the proportion of alcohol-related injuries compared to patient BAC. Implications: Given the current role of administrative data in the surveillance of alcohol-related injuries, these findings may have significant implications for the implementation of cost-effective strategies for preventing alcohol-related injuries.

This paper presents a comprehensive analysis of the pollutant emissions from electrical generation facilities reported to Australia's National Pollutant Inventory (NPI). The data, in terms of pollutant intensity with respect to generation capacity and fuel source, show significant variability. Based on reported data, the dominant pathway and environmental segment for emissions is point-source air emissions. Surprisingly, pollutant emissions from power stations are generally a very small fraction of Australia's facility and diffuse emissions, except for F, HCl, NO(x), PM2.5, SO2 and H2SO4 (where it constitutes between 30 and 90% of emissions). In general, natural gas and diesel facilities have higher organic pollutant intensities, while black and brown coal have higher metal/metalloid pollutant intensities and there is a wide variability for inorganic pollutant intensities. When examining pollutant intensities with respect to capacity, there is very little evidence to show that increased scale leads to more efficient operation or lower pollutant intensity. Another important finding is that the pollutant loads associated with transfers and reuse are substantial, and often represent most of the reported pollutants from a given generation facility. Finally, given the issues identified with the NPI data and its use, some possible improvements include the following: (i) linking site generation data to NPI data (especially generation data, i.e., MWh); (ii) better validation and documentation of emissions factors, especially the methods used to derive and report estimates to the NPI; (iii) using NPI data to undertake comparative life cycle impact assessment studies of different power stations and fuel/energy sources, or even intensive industrial regions (especially from a toxicity perspective) and (iv) linking NPI data in a given region to ongoing environmental monitoring, so that loads can be linked to concentrations for particular pollutants and the relevant guidelines (e.g., air, water, human health). Pollutant inventory systems are clearly valuable tools in understanding pollution burdens and ongoing analysis of the growing body of data should help to further improve environmental and public health outcomes. Overall, this study provides a valuable insight into the current status of pollutant intensities from Australia's electrical generation facilities and should be a valuable benchmark for future studies and international comparisons.

Despite overall progress toward achieving the Millennium Development Goals, large health discrepancies persist between developed and developing countries. The world is rapidly changing and the influences of societal change and climate change will disproportionately affect the world's most vulnerable populations, thus exacerbating current inequities. Current development strategies do not adequately address these disproportionate impacts of environmental exposures. The aim of this study was to propose a new framework to address the health consequences of environmental exposures beyond 2015. This framework is transdisciplinary and precautionary. It is based on identifying social and economic determinants of health, strengthening primary health systems, and improving the health of vulnerable populations. It incorporates deliberate plans for assessment and control of avoidable environmental exposures. It sets specific, measurable targets for health and environmental improvement.

Joven in OpenAIREBackground: Variable environmental temperature changes have affected different population groups; however, limited empirical studies in nursing have focused on older adults. One of the most pressing issues of today is climate change, where nurses should explore and advance their roles. Objective: This study aimed to explore and describe the experience of older adults with temperature variations as a consequence of climate change and its impact on mental health. Methods: Descriptive phenomenological approach was used in this study. Purposeful sampling was used in selecting key informants, while individual telephone interviews and videoconferencing platforms using a semi-structured interview guide were used as the data gathering method to gather key informants’ accounts of their experiences. Prior to data collection, authors bracket their preconceived beliefs and values regarding the phenomenon at hand. Colaizzi’s seven-step method of data analysis combined with the use of qualitative data analysis software was used to develop the themes and sub-themes. Results: Eleven key informants aged 60-77 participated in this study. Sixty significant statements on temperature variations and mental health were extracted, while forty-one meanings were formulated from the significant statements. Important meanings were grouped into a cluster of themes which built the three major themes and seven sub-themes. The major themes are Theme 1: Active recollection and recognition of their experience; Theme 2: Symptoms of physical, social, mental, and cognitive aspects transformation; and Theme 3: Establishing adaptive mechanisms. The themes reflect a mental or cognitive process of recognizing and understanding their experiences, experiencing their impacts, developing adaptive mechanisms, and showing characteristics of resilience. Conclusion: The themes and sub-themes developed in this study showed that despite their vulnerability, older adults were able to implement adaptive mechanisms and showed signs of resiliency. Temperature variations brought limitations or adjustments to the physical, social, mental, and cognitive aspects of the key informants. Further studies should be conducted in other population groups and various geographical locations to identify sources of adaptive mechanisms and resilience to temperature variations and climate change among older adults. The findings should be used in developing health programs and crafting policies.

Amjad Anvari-Moghaddam in OpenAIREThe COVID-19 pandemic affects all of society and hinders day-to-day activities from a straightforward perspective. The pandemic has an influential impact on almost everything and the characteristics of the pandemic remain unclear. This ultimately leads to ineffective strategic planning to manage the pandemic. This study aims to elucidate the typical pandemic characteristics in line with various temporal phases and its associated measures that proved effective in controlling the pandemic. Besides, an insight into diverse country's approaches towards pandemic and their consequences is provided in brief. Understanding the role of technologies in supporting humanity gives new perspectives to effectively manage the pandemic. Such role of technologies is expressed from the viewpoint of seamless connectivity, rapid communication, mobility, technological influence in healthcare, digitalization influence, surveillance and security, Artificial Intelligence (AI), and Internet of Things (IoT). Furthermore, some insightful scenarios are framed where the full-fledged implementation of technologies is assumed, and the reflected pandemic impacts in such scenarios are analyzed. The framed scenarios revolve around the digitalized energy sector, an enhanced supply chain system with effective customer-retailer relationships to support the city during the pandemic scenario, and an advanced tracking system for containing virus spread. The study is further extended to frame revitalization strategies to highlight the expertise where significant attention needs to be provided in the post-pandemic period as well as to nurture sustainable development. Finally, the current pandemic scenario is analyzed in terms of occurred changes and is mapped into SWOT factors. Using Fuzzy Technique for Order of Preference by Similarity to Ideal Solution based Multi-Criteria Decision Analysis, these SWOT factors are analyzed to determine where prioritized efforts are needed to focus so as to traverse towards sustainable cities. The results indicate that the enhanced crisis management ability and situational need to restructure the economic model emerges to be the most-significant SWOT factor that can ultimately support humanity for making the cities sustainable.

Peng Jia in OpenAIRE(1) Background: Although the health effects of future climate change have been examined in previous studies, few have considered additive impacts of population expansion, ageing, and adaptation. We aimed to quantify the future heat-related years of life lost (YLLs) under different Representative Concentration Pathways (RCP) scenarios and global-scale General Circulation Models (GCMs), and further to examine relative contributions of population expansion, ageing, and adaptation on these projections. (2) Methods: We used downscaled and bias-corrected projections of daily temperature from 27 GCMs under RCP2.6, 4.5, and 8.5 scenarios to quantify the potential annual heat-related YLLs in Guangzhou, China in the 2030s, 2060s, and 2090s, compared to those in the 1980s as a baseline. We also explored the modification effects of a range of population expansion, ageing, and adaptation scenarios on the heat-related YLLs. (3) Results: Global warming, particularly under the RCP8.5 scenario, would lead to a substantial increase in the heat-related YLLs in the 2030s, 2060s, and 2090s for the majority of the GCMs. For the total population, the annual heat-related YLLs under the RCP8.5 in the 2030s, 2060s, and 2090s were 2.2, 7.0, and 11.4 thousand, respectively. The heat effects would be significantly exacerbated by rapid population expansion and ageing. However, substantial heat-related YLLs could be counteracted by the increased adaptation (75% for the total population and 20% for the elderly). (4) Conclusions: The rapid population expansion and ageing coinciding with climate change may present an important health challenge in China, which, however, could be partially counteracted by the increased adaptation of individuals.

Francis J. Sullivan in OpenAIREThe ocean provides resources key to human health and well-being, including food, oxygen, livelihoods, blue spaces, and medicines. The global threat to these resources posed by accelerating ocean acidification is becoming increasingly evident as the world’s oceans absorb carbon dioxide emissions. While ocean acidification was initially perceived as a threat only to the marine realm, here we argue that it is also an emerging human health issue. Specifically, we explore how ocean acidification affects the quantity and quality of resources key to human health and well-being in the context of: (1) malnutrition and poisoning, (2) respiratory issues, (3) mental health impacts, and (4) development of medical resources. We explore mitigation and adaptation management strategies that can be implemented to strengthen the capacity of acidifying oceans to continue providing human health benefits. Importantly, we emphasize that the cost of such actions will be dependent upon the socioeconomic context; specifically, costs will likely be greater for socioeconomically disadvantaged populations, exacerbating the current inequitable distribution of environmental and human health challenges. Given the scale of ocean acidification impacts on human health and well-being, recognizing and researching these complexities may allow the adaptation of management such that not only are the harms to human health reduced but the benefits enhanced.

Beard, M. in OpenAIREHepatitis C virus (HCV) is one of the main etiological factors responsible for liver disease worldwide. It has been estimated that there are over 170 million people infected with HCV worldwide. Of these infected individuals, approximately 75% will go on to develop a life long necroinflammatory liver disease, which over decades, can result in serious complications, such as cirrhosis and hepatocellular carcinoma. Currently there is no effective vaccine and whilst antiviral therapies have been improved, they are still only effective in approximately 50% of individuals. HCV infection stands as a major cause of global morbidity and suffering, and places a significant burden on health systems. The second highest cause of liver disease in the western world is alcoholic liver disease. Frequently, HCV infected individuals consume alcohol, and the combined effect of HCV and alcohol consumption is deleterious for both liver disease and response to treatment. This review discusses the impact of alcohol metabolism on HCV replication and the negative impact on interferon (IFN)-alpha treatment, with a particular focus on how alcohol and HCV act synergistically to increase oxidative stress, ultimately leading to exacerbated liver disease and a reduction in the efficacy of IFN-alpha treatment. A better understanding of the complicated mechanisms at play in hepatocytes infected with HCV and metabolizing alcohol will hopefully provide better treatment options for chronic hepatitis C individuals that consume alcohol.
