To reorient food systems to ensure they deliver healthy diets that protect against multiple forms of malnutrition and diet-related disease and safeguard the environment, ecosystems, and natural resources, there is a need for better governance and accountability. However, decision-makers are often in the dark on how to navigate their food systems to achieve these multiple outcomes. Even where there is sufficient data to describe various elements, drivers, and outcomes of food systems, there is a lack of tools to assess how food systems are performing. This paper presents a diagnostic methodology for 39 indicators representing food supply, food environments, nutrition outcomes, and environmental outcomes that offer cutoffs to assess performance of national food systems. For each indicator, thresholds are presented for unlikely, potential, or likely challenge areas. This information can be used to generate actions and decisions on where and how to intervene in food systems to improve human and planetary health. A global assessment and two country case studies—Greece and Tanzania—illustrate how the diagnostics could spur decision options available to countries.

Gill;AbstractLivestock are a critically important component of the food system, although the sector needs a profound transformation to ensure that it contributes to a rapid transition towards sustainable food systems. This chapter reviews and synthesises the evidence available on changes in demand for livestock products in the last few decades, and the multiple socio-economic roles that livestock have around the world. We also describe the nutrition, health, and environmental impacts for which the sector is responsible. We propose eight critical actions for transitioning towards a more sustainable operating space for livestock. (1) Facilitate shifts in the consumption of animal source foods (ASF), recognising that global reductions will be required, especially in communities with high consumption levels, while promoting increased levels in vulnerable groups, including the undernourished, pregnant women and the elderly. (2) Continue work towards the sustainable intensification of livestock systems, paying particular attention to animal welfare, food-feed competition, blue water use, disease transmission and perverse economic incentives. (3) Embrace the potential of circularity in livestock systems as a way of partially decoupling livestock from land. (4) Adopt practices that lead to the direct or indirect mitigation of greenhouse gases. (5) Adopt some of the vast array of novel technologies at scale and design incentive mechanisms for their rapid deployment. (6) Diversify the protein sources available for human consumption and feed, focusing on the high-quality alternative protein sources that have lower environmental impacts. (7) Tackle antimicrobial resistance effectively through a combination of technology and new regulations, particularly for the fast-growing poultry and pork sectors and for feedlot operations. (8) Implement true cost of food and true-pricing approaches to ASF consumption.

Poor nutrition and health outcomes of populations have stimulated programs and policies to reduce the intake of salt, added sugar, and unhealthy fats and increase healthy nutrients and ingredients to meet recommended targets of a healthy diet. Alongside promoting theconsumption of whole, nutritious foods (e.g., fruits, vegetables, and whole grains), reformulation, including fortification of processed foods, has been utilized by food industry manufacturers to contribute to improvingdiets and aligning with national dietary guidelines. This paper summarizes a literature review and twenty semistructured interviews with experts on food product reformulation and fortification to highlight the challenges,limitations, and opportunities for increasing their effectiveness. While studies have shown that reformulation could have beneficial public health impacts, such as iodized salt, there are a dearth of rigorous evaluations, particularly for some types of reformulations. Importantly, some evidence suggests that ultra-processing has significant adverse health effects independently of nutrient adequacy. To improve population health, reformulation should be complemented with a range of approaches, including food taxes and subsidies, public foodprocurement, restrictions on food advertising and marketing, front-of-pack labeling, and changes to food environments that improve availability, affordability, and demand for whole and minimally processed foods

Household air pollution (HAP) from solid cooking fuels continues to affect 600 million people in China and has been associated with high blood pressure. The role of diet in HAP-associated high blood pressure has yet to be evaluated in China. The aim of this study was to estimate the impact of cooking fuel on change in blood pressure and evaluate whether intake of antioxidant- and omega-3 fatty acid-rich foods (fruits, vegetables, and seafood) attenuates any adverse effects of solid fuel use on blood pressure. We analyzed longitudinal data collected between 1991 and 2011 from nonpregnant women aged 18 to 80 years living in rural areas of China. We used linear mixed effects models to estimate the association between cooking fuel (coal or wood versus clean [electric or liquid petroleum gas]) and blood pressure. Possible mediation of the fuel effect by diet was assessed by the difference method. A total of 6671 women were included in this study. Women less than 40 years of age cooking with cleaner fuels over time had lower rates of change in systolic blood pressure compared to women cooking with coal (p = 0.004), and this effect was not mediated by dietary intake. Associations between fuel use and change in diastolic blood pressure were not significant. These findings lend further support for there being a direct effect of reducing HAP on improvements in blood pressure, independent of concurrent dietary intake.

Household air pollution (HAP) from solid cooking fuels continues to affect 600 million people in China and has been associated with high blood pressure. The role of diet in HAP-associated high blood pressure has yet to be evaluated in China. The aim of this study was to estimate the impact of cooking fuel on change in blood pressure and evaluate whether intake of antioxidant- and omega-3 fatty acid-rich foods (fruits, vegetables, and seafood) attenuates any adverse effects of solid fuel use on blood pressure. The framework and indicators were then mapped to data collected by consumer and vendor surveys using the Urban Food Systems Assessment Tool (UFSAT) in Ahmedabad (India), Pune (India), Kathmandu (Nepal), and Pokhara (Nepal).We have expanded the sustainability domain within food environments to include consumer travel to food vendors, the presence of food delivery services, policies related to sustainability, vendor food waste, vendor plastic use, vendor utility usage, vendor recycling and waste management practices, and food packaging. Mapping the framework to existing data from 4 cities in South Asia, we found variations in food environment sustainability indicators, particularly regarding consumer transportation to food vendors, the presence of delivery services, and food waste.Although the majority of food environment research focuses on the availability and affordability of healthy foods, there is an urgent need to understand better how aspects of food environments contribute to environmental goals. When mapping the framework to existing food systems data, we found gaps in data on environmental sustainability in food environments and variation in indicators across settings.

Global and local food system transformation is necessary in order to ensure the delivery of healthy, safe, and nutritious foods in both sustainable and equitable ways. Food systems are complex entities that affect diets, human health, and a range of other outcomes including economic growth, natural resource and environmental resiliency, and sociocultural factors. However, food systems contribute to and are vulnerable to ongoing climate and environmental changes that threaten their sustainability. Although there has been increased focus on this topic in recent years, many gaps in our knowledge persist on the relation between environmental factors, food systems, and nutritional outcomes. In this article, we summarize this emerging field and describe what innovative nutrition research is needed in order to bring about food policy changes in the era of climate disruption and environmental degradation.

Jones; Jessica Fanzo;Dramatic improvements in data availability and quality are needed to meet the challenge of monitoring and analyzing food systems, so that appropriate policies and actions to improve human and planetary health can be identified and data-informed accountability mechanisms put in place to strengthen food systems governance. Studying food systems is complex due to diverse actors and interlinking processes that operate on multiple spatial and temporal scales, and their multiple outcomes, which may be subject to hidden feedback mechanisms and tradeoffs. However, descriptive research to characterize food system components and make comparisons across geography, income groups, and population groups is an important foundation. The first part of this article details a series of critical data gaps and limitations that are currently hindering food systems learning and accountability, also comparing these gaps across regions and income groups. The second part of the article introduces the Food Systems Dashboard, a new data visualization tool that aims to improve access to and usage of food systems-related data, thus strengthening the data value chain and better informing policies and actions intended to improve diets, nutrition, livelihoods, and environmental sustainability.

To reorient food systems to ensure they deliver healthy diets that protect against multiple forms of malnutrition and diet-related disease and safeguard the environment, ecosystems, and natural resources, there is a need for better governance and accountability. However, decision-makers are often in the dark on how to navigate their food systems to achieve these multiple outcomes. Even where there is sufficient data to describe various elements, drivers, and outcomes of food systems, there is a lack of tools to assess how food systems are performing. This paper presents a diagnostic methodology for 39 indicators representing food supply, food environments, nutrition outcomes, and environmental outcomes that offer cutoffs to assess performance of national food systems. For each indicator, thresholds are presented for unlikely, potential, or likely challenge areas. This information can be used to generate actions and decisions on where and how to intervene in food systems to improve human and planetary health. A global assessment and two country case studies—Greece and Tanzania—illustrate how the diagnostics could spur decision options available to countries.

Gill;AbstractLivestock are a critically important component of the food system, although the sector needs a profound transformation to ensure that it contributes to a rapid transition towards sustainable food systems. This chapter reviews and synthesises the evidence available on changes in demand for livestock products in the last few decades, and the multiple socio-economic roles that livestock have around the world. We also describe the nutrition, health, and environmental impacts for which the sector is responsible. We propose eight critical actions for transitioning towards a more sustainable operating space for livestock. (1) Facilitate shifts in the consumption of animal source foods (ASF), recognising that global reductions will be required, especially in communities with high consumption levels, while promoting increased levels in vulnerable groups, including the undernourished, pregnant women and the elderly. (2) Continue work towards the sustainable intensification of livestock systems, paying particular attention to animal welfare, food-feed competition, blue water use, disease transmission and perverse economic incentives. (3) Embrace the potential of circularity in livestock systems as a way of partially decoupling livestock from land. (4) Adopt practices that lead to the direct or indirect mitigation of greenhouse gases. (5) Adopt some of the vast array of novel technologies at scale and design incentive mechanisms for their rapid deployment. (6) Diversify the protein sources available for human consumption and feed, focusing on the high-quality alternative protein sources that have lower environmental impacts. (7) Tackle antimicrobial resistance effectively through a combination of technology and new regulations, particularly for the fast-growing poultry and pork sectors and for feedlot operations. (8) Implement true cost of food and true-pricing approaches to ASF consumption.

Poor nutrition and health outcomes of populations have stimulated programs and policies to reduce the intake of salt, added sugar, and unhealthy fats and increase healthy nutrients and ingredients to meet recommended targets of a healthy diet. Alongside promoting theconsumption of whole, nutritious foods (e.g., fruits, vegetables, and whole grains), reformulation, including fortification of processed foods, has been utilized by food industry manufacturers to contribute to improvingdiets and aligning with national dietary guidelines. This paper summarizes a literature review and twenty semistructured interviews with experts on food product reformulation and fortification to highlight the challenges,limitations, and opportunities for increasing their effectiveness. While studies have shown that reformulation could have beneficial public health impacts, such as iodized salt, there are a dearth of rigorous evaluations, particularly for some types of reformulations. Importantly, some evidence suggests that ultra-processing has significant adverse health effects independently of nutrient adequacy. To improve population health, reformulation should be complemented with a range of approaches, including food taxes and subsidies, public foodprocurement, restrictions on food advertising and marketing, front-of-pack labeling, and changes to food environments that improve availability, affordability, and demand for whole and minimally processed foods

Household air pollution (HAP) from solid cooking fuels continues to affect 600 million people in China and has been associated with high blood pressure. The role of diet in HAP-associated high blood pressure has yet to be evaluated in China. The aim of this study was to estimate the impact of cooking fuel on change in blood pressure and evaluate whether intake of antioxidant- and omega-3 fatty acid-rich foods (fruits, vegetables, and seafood) attenuates any adverse effects of solid fuel use on blood pressure. We analyzed longitudinal data collected between 1991 and 2011 from nonpregnant women aged 18 to 80 years living in rural areas of China. We used linear mixed effects models to estimate the association between cooking fuel (coal or wood versus clean [electric or liquid petroleum gas]) and blood pressure. Possible mediation of the fuel effect by diet was assessed by the difference method. A total of 6671 women were included in this study. Women less than 40 years of age cooking with cleaner fuels over time had lower rates of change in systolic blood pressure compared to women cooking with coal (p = 0.004), and this effect was not mediated by dietary intake. Associations between fuel use and change in diastolic blood pressure were not significant. These findings lend further support for there being a direct effect of reducing HAP on improvements in blood pressure, independent of concurrent dietary intake.

The framework and indicators were then mapped to data collected by consumer and vendor surveys using the Urban Food Systems Assessment Tool (UFSAT) in Ahmedabad (India), Pune (India), Kathmandu (Nepal), and Pokhara (Nepal).We have expanded the sustainability domain within food environments to include consumer travel to food vendors, the presence of food delivery services, policies related to sustainability, vendor food waste, vendor plastic use, vendor utility usage, vendor recycling and waste management practices, and food packaging. Mapping the framework to existing data from 4 cities in South Asia, we found variations in food environment sustainability indicators, particularly regarding consumer transportation to food vendors, the presence of delivery services, and food waste.Although the majority of food environment research focuses on the availability and affordability of healthy foods, there is an urgent need to understand better how aspects of food environments contribute to environmental goals. When mapping the framework to existing food systems data, we found gaps in data on environmental sustainability in food environments and variation in indicators across settings.

Global and local food system transformation is necessary in order to ensure the delivery of healthy, safe, and nutritious foods in both sustainable and equitable ways. Food systems are complex entities that affect diets, human health, and a range of other outcomes including economic growth, natural resource and environmental resiliency, and sociocultural factors. However, food systems contribute to and are vulnerable to ongoing climate and environmental changes that threaten their sustainability. Although there has been increased focus on this topic in recent years, many gaps in our knowledge persist on the relation between environmental factors, food systems, and nutritional outcomes. In this article, we summarize this emerging field and describe what innovative nutrition research is needed in order to bring about food policy changes in the era of climate disruption and environmental degradation.

Jones; Jessica Fanzo;Dramatic improvements in data availability and quality are needed to meet the challenge of monitoring and analyzing food systems, so that appropriate policies and actions to improve human and planetary health can be identified and data-informed accountability mechanisms put in place to strengthen food systems governance. Studying food systems is complex due to diverse actors and interlinking processes that operate on multiple spatial and temporal scales, and their multiple outcomes, which may be subject to hidden feedback mechanisms and tradeoffs. However, descriptive research to characterize food system components and make comparisons across geography, income groups, and population groups is an important foundation. The first part of this article details a series of critical data gaps and limitations that are currently hindering food systems learning and accountability, also comparing these gaps across regions and income groups. The second part of the article introduces the Food Systems Dashboard, a new data visualization tool that aims to improve access to and usage of food systems-related data, thus strengthening the data value chain and better informing policies and actions intended to improve diets, nutrition, livelihoods, and environmental sustainability.
