The global goal to end hunger requires the interpretation of problems and change across multiple domains to create the scope for collaboration, learning, and impactful research. We facilitated a workshop aimed at understanding how stakeholders problematize sustainable diet transition (SDT) among a previously marginalized social group. Using the systems thinking approach, three sub-systems, namely access to dietary diversity, sustainable beneficiation of natural capital, and ‘food choice for well-being’, highlighted the main forces governing the current context, and future interventions of the project. Moreover, when viewed as co-evolving processes within the multi-level perspective, our identified microlevel leverage points—multi-faceted literacy, youth empowerment, deliberative policymaking, and promotion of sustainable diet aspirations—can be linked and developed through existing national macro-level strategies. Thus, co-designing to problematize transformational SDT, centered on an interdisciplinary outlook and informational governance, could streamline research implementation outcomes to re-structure socio-technical sectors and reconnect people to nature-based solutions. Such legitimate aspirations could be relevant in countries bearing complex socio-political legacies and bridge the local–global goals coherently. This work provides a collaborative framework required to develop impact-driven activities needed to inform evidence-based policies on sustainable diets.

Previous research confirmed the environmental benefits of carbon savings from international virtual conferences. This paper adds the social and economic dimensions by using a combination of qualitative and quantitative methods to measure the constraints and benefits for personal development, economic costs, efficiency and team learning of holding international virtual conferences (VCs). Using the Sustainable and Healthy Food Systems (SHEFS) research programme as a case study, we analysed VC participant survey data to identify strengths, weaknesses, opportunities, and threats of VCs. We estimated ‘saved’ GHG emissions, costs, and time, of using VCs as an alternative for a planned in-person meeting in Chennai, India. Hosting VCs reduced North–South, gender, and researcher inclusivity concerns, financial and travelling time costs, and substantially reduced emissions. For one international meeting with 107 participants, changing to a virtual format reduced the per capita GHG emissions to half the annual global average, and avoided 60% of travel costs. The benefits of VCs outweighed weaknesses. The main strengths were inclusivity and access, with 20% more early/mid-career researchers attending. This study identified opportunities for international research partnerships to mitigate their carbon footprint (environmental benefit) and enhance inclusivity of early/mid-career, women and Global South participants (social benefit), whilst continuing to deliver effective collaborative research meetings (economic benefit). In doing so, we present a holistic view of sustainability opportunities for virtual collaboration.

Abstract Climate change adaptation responses are being developed and delivered in many parts of the world in the absence of detailed knowledge of their effects on public health. Here we present the results of a systematic review of peer-reviewed literature reporting the effects on health of climate change adaptation responses in low- and middle-income countries (LMICs). The review used the ‘Global Adaptation Mapping Initiative’ database (comprising 1682 publications related to climate change adaptation responses) that was constructed through systematic literature searches in Scopus, Web of Science and Google Scholar (2013–2020). For this study, further screening was performed to identify studies from LMICs reporting the effects on human health of climate change adaptation responses. Studies were categorised by study design and data were extracted on geographic region, population under investigation, type of adaptation response and reported health effects. The review identified 99 studies (1117 reported outcomes), reporting evidence from 66 LMICs. Only two studies were ex ante formal evaluations of climate change adaptation responses. Papers reported adaptation responses related to flooding, rainfall, drought and extreme heat, predominantly through behaviour change, and infrastructural and technological improvements. Reported (direct and intermediate) health outcomes included reduction in infectious disease incidence, improved access to water/sanitation and improved food security. All-cause mortality was rarely reported, and no papers were identified reporting on maternal and child health. Reported maladaptations were predominantly related to widening of inequalities and unforeseen co-harms. Reporting and publication-bias seems likely with only 3.5% of all 1117 health outcomes reported to be negative. Our review identified some evidence that climate change adaptation responses may have benefits for human health but the overall paucity of evidence is concerning and represents a major missed opportunity for learning. There is an urgent need for greater focus on the funding, design, evaluation and standardised reporting of the effects on health of climate change adaptation responses to enable evidence-based policy action.

AbstractWith rising demand for food and the threats posed by climate change, The Gambia faces significant challenges in ensuring sufficient and nutritious food for its population. To address these challenges, there is a need to increase domestic food production while limiting deforestation and land degradation. In this study, we modified the FABLE Calculator, a food and land-use system model, to focus on The Gambia to simulate scenarios for future food demand and increasing domestic food production. We considered the impacts of climate change on crops, the adoption of climate change adaptation techniques, as well as the potential of enhanced fertiliser use and irrigation to boost crop productivity, and assessed whether these measures would be sufficient to meet the projected increase in food demand. Our results indicate that domestic food production on existing cropland will not be sufficient to meet national food demand by 2050, leading to a significant supply–demand gap. However, investments in fertiliser availability and the development of sustainable irrigation infrastructure, coupled with climate change adaptation strategies like the adoption of climate-resilient crop varieties and optimised planting dates, could halve this gap. Addressing the remaining gap will require additional strategies, such as increasing imports, expanding cropland, or prioritising the production of domestic food crops over export crops. Given the critical role imports play in The Gambia’s food supply, it is essential to ensure a robust flow of food imports by diversifying partners and addressing regional trade barriers. Our study highlights the urgent need for sustained investment and policy support to enhance domestic food production and food imports to secure sufficient and healthy food supplies amidst growing demand and climate change challenges.

Prentice; Rosemary Green;Abstract Facilitating dietary change is pivotal to improving population health, increasing food system resilience, and minimizing adverse impacts on the environment, but assessment of the current ‘status-quo’ and identification of bottlenecks for improvement has been lacking to date. We assessed deviation of the Gambian diet from the EAT-Lancet guidelines for healthy and sustainable diets and identified leverage points to improve nutritional and planetary health. We analysed the 2015/16 Gambian Integrated Household Survey dataset comprising food consumption data from 12 713 households. Consumption of different food groups was compared against the EAT-Lancet reference diet targets to assess deviation from the guidelines. We computed a ‘sustainable and healthy diet index (SHDI)’ based on deviation of different food groups from the EAT-Lancet recommendations and modelled the socio-economic and geographic determinants of households that achieved higher scores on this index, using multivariable mixed effects regression. The average Gambian diet had very low adherence to EAT-Lancet recommendations. The diet was dominated by refined grains and added sugars which exceeded the recommendations. SHDI scores for nutritionally important food groups such as fruits, vegetables, nuts, dairy, poultry, and beef and lamb were low. Household characteristics associated with higher SHDI scores included: being a female-headed household, having a relatively small household size, having a schooled head of the household, having a high wealth index, and residing in an urban settlement. Furthermore, diets reported in the dry season and households with high crop production diversity showed increased adherence to the targets. While average Gambian diets include lower amounts of food groups with harmful environmental footprint, they are also inadequate in healthy food groups and are high in sugar. There are opportunities to improve diets without increasing their environmental footprint by focusing on the substitution of refined grains by wholegrains, reducing sugar and increasing fruit and vegetables consumption.

However, for many countries, the optimal composition of locally contextualized healthy and sustainable diets is unknown. The Gambia is vulnerable to climate-change-induced future water scarcity which may affect crop yields and the ability to supply healthy diets. This study identifies potential shifts in Gambian diets that could make diets healthier and reduce the associated agricultural water footprint (WF), and assesses the cost and affordability implications of such dietary changes. Gambian Integrated Household Survey (IHS) food consumption data was combined with market prices, food expenditure and agricultural WF data. Current dietary patterns were compared with World Health Organization (WHO) dietary guidelines and optimized using linear programming to identify least-cost diets that met nutrition recommendations and reduced agricultural water use. Optimization scenarios explored the maximum reduction in green water use that could be achieved with ‘culturally-acceptable’ dietary shifts, and the magnitude of shifts required to maintain green water use at current levels. On average, current diets provide adequate energy and have appropriate macronutrient composition. However, only 14% of households consume enough fruit and vegetables (F&Vs), and consumption of added sugars exceeds recommendations. With ‘culturally-acceptable’ changes in consumption, agricultural water use could decrease by 10%–13% or increase by 9%, depending on the baseline dietary pattern. Extreme dietary shifts will be required to maintain water use at 2015 levels with projected population growth. To meet WHO recommendations, dietary costs would increase by 43% compared to the current baseline. Healthy and green water-saving diets would require 48%–63% of average household expenditure to purchase, which is unaffordable for almost half of the population. F&Vs alone account for 31%–40% of the cost of optimized diets compared to 12% of current diets. Dietary modification has the potential to improve the nutritional quality of Gambian diets while reducing agricultural water use, but the required changes are likely to be unaffordable for a large proportion of the population. Improving availability and affordability of nutritious foods—particularly F&Vs—will be crucial for the accessibility of healthy and sustainable diets in the Gambian population.

There is evidence that environmental change will reduce the yields of starchy staple crops, but impacts on (nonstaple) vegetables and legumes—important constituents of healthy diets—remain largely unknown. We systematically reviewed the available published evidence from experimental studies on the impact of environmental changes on yields and nutritional quality of (nonstaple) vegetables and legumes and found that environmental change would have a negative impact on yields without suitable responses from the agricultural sector. An enhanced understanding of the scale of environmental impacts on agricultural production is essential for the development of effective strategies to protect global population health.

Reducing food system emissions is an essential component of climate change mitigation, and a system-wide approach, including production, processing, trade and demand-side transformations, will be needed. Long-term analysis of greenhouse gas (GHG) emissions of food supply is crucial for informing this transformation, and understanding the processes contributing to existing trends can reveal opportunities for future mitigation strategies. To address these needs we used data on food supply, trade and emission intensity to quantify changes in GHG emissions between 1986 and 2017 resulting from food supply in the United Kingdom (UK). Uniquely, the relative contributions of supply-side and demand-side changes to historical trends in food emissions were assessed, and the gap between current UK food consumption and EAT-Lancet recommended diets was used to estimate the additional GHG emission reductions that could be achieved by shifting to the Planetary Health Diet (PHD). It was estimated that in the UK, per-capita GHG emissions from food fell by 32% (from 4.6 tCO2eq/capita to 3.1 tCO2eq/capita) between 1986 and 2017. Of this 32% reduction, 21% was due to supply-side changes (a fall in emission intensity per unit of production due to increased efficiency of farming practices), 10% was due to demand-side changes (including dietary change and waste reduction), and 2% was due to changing trade patterns. Relative to the PHD, however, the average UK citizen still greatly over-consumes beef, lamb and pork, tubers and starchy vegetables and dairy products, and under-consumes vegetables, nuts, and legumes. It was estimated that by adopting the PHD, UK per capita food emissions could be reduced by a further 42% to 1.8 tCO2eq/capita. These results expose the historic contributions of both supply- and demand-side changes to reductions in GHG emissions from food, and highlight the underutilised potential of dietary change in contributing to mitigation of GHG emissions from food.

The global goal to end hunger requires the interpretation of problems and change across multiple domains to create the scope for collaboration, learning, and impactful research. We facilitated a workshop aimed at understanding how stakeholders problematize sustainable diet transition (SDT) among a previously marginalized social group. Using the systems thinking approach, three sub-systems, namely access to dietary diversity, sustainable beneficiation of natural capital, and ‘food choice for well-being’, highlighted the main forces governing the current context, and future interventions of the project. Moreover, when viewed as co-evolving processes within the multi-level perspective, our identified microlevel leverage points—multi-faceted literacy, youth empowerment, deliberative policymaking, and promotion of sustainable diet aspirations—can be linked and developed through existing national macro-level strategies. Thus, co-designing to problematize transformational SDT, centered on an interdisciplinary outlook and informational governance, could streamline research implementation outcomes to re-structure socio-technical sectors and reconnect people to nature-based solutions. Such legitimate aspirations could be relevant in countries bearing complex socio-political legacies and bridge the local–global goals coherently. This work provides a collaborative framework required to develop impact-driven activities needed to inform evidence-based policies on sustainable diets.

Previous research confirmed the environmental benefits of carbon savings from international virtual conferences. This paper adds the social and economic dimensions by using a combination of qualitative and quantitative methods to measure the constraints and benefits for personal development, economic costs, efficiency and team learning of holding international virtual conferences (VCs). Using the Sustainable and Healthy Food Systems (SHEFS) research programme as a case study, we analysed VC participant survey data to identify strengths, weaknesses, opportunities, and threats of VCs. We estimated ‘saved’ GHG emissions, costs, and time, of using VCs as an alternative for a planned in-person meeting in Chennai, India. Hosting VCs reduced North–South, gender, and researcher inclusivity concerns, financial and travelling time costs, and substantially reduced emissions. For one international meeting with 107 participants, changing to a virtual format reduced the per capita GHG emissions to half the annual global average, and avoided 60% of travel costs. The benefits of VCs outweighed weaknesses. The main strengths were inclusivity and access, with 20% more early/mid-career researchers attending. This study identified opportunities for international research partnerships to mitigate their carbon footprint (environmental benefit) and enhance inclusivity of early/mid-career, women and Global South participants (social benefit), whilst continuing to deliver effective collaborative research meetings (economic benefit). In doing so, we present a holistic view of sustainability opportunities for virtual collaboration.

Abstract Climate change adaptation responses are being developed and delivered in many parts of the world in the absence of detailed knowledge of their effects on public health. Here we present the results of a systematic review of peer-reviewed literature reporting the effects on health of climate change adaptation responses in low- and middle-income countries (LMICs). The review used the ‘Global Adaptation Mapping Initiative’ database (comprising 1682 publications related to climate change adaptation responses) that was constructed through systematic literature searches in Scopus, Web of Science and Google Scholar (2013–2020). For this study, further screening was performed to identify studies from LMICs reporting the effects on human health of climate change adaptation responses. Studies were categorised by study design and data were extracted on geographic region, population under investigation, type of adaptation response and reported health effects. The review identified 99 studies (1117 reported outcomes), reporting evidence from 66 LMICs. Only two studies were ex ante formal evaluations of climate change adaptation responses. Papers reported adaptation responses related to flooding, rainfall, drought and extreme heat, predominantly through behaviour change, and infrastructural and technological improvements. Reported (direct and intermediate) health outcomes included reduction in infectious disease incidence, improved access to water/sanitation and improved food security. All-cause mortality was rarely reported, and no papers were identified reporting on maternal and child health. Reported maladaptations were predominantly related to widening of inequalities and unforeseen co-harms. Reporting and publication-bias seems likely with only 3.5% of all 1117 health outcomes reported to be negative. Our review identified some evidence that climate change adaptation responses may have benefits for human health but the overall paucity of evidence is concerning and represents a major missed opportunity for learning. There is an urgent need for greater focus on the funding, design, evaluation and standardised reporting of the effects on health of climate change adaptation responses to enable evidence-based policy action.

AbstractWith rising demand for food and the threats posed by climate change, The Gambia faces significant challenges in ensuring sufficient and nutritious food for its population. To address these challenges, there is a need to increase domestic food production while limiting deforestation and land degradation. In this study, we modified the FABLE Calculator, a food and land-use system model, to focus on The Gambia to simulate scenarios for future food demand and increasing domestic food production. We considered the impacts of climate change on crops, the adoption of climate change adaptation techniques, as well as the potential of enhanced fertiliser use and irrigation to boost crop productivity, and assessed whether these measures would be sufficient to meet the projected increase in food demand. Our results indicate that domestic food production on existing cropland will not be sufficient to meet national food demand by 2050, leading to a significant supply–demand gap. However, investments in fertiliser availability and the development of sustainable irrigation infrastructure, coupled with climate change adaptation strategies like the adoption of climate-resilient crop varieties and optimised planting dates, could halve this gap. Addressing the remaining gap will require additional strategies, such as increasing imports, expanding cropland, or prioritising the production of domestic food crops over export crops. Given the critical role imports play in The Gambia’s food supply, it is essential to ensure a robust flow of food imports by diversifying partners and addressing regional trade barriers. Our study highlights the urgent need for sustained investment and policy support to enhance domestic food production and food imports to secure sufficient and healthy food supplies amidst growing demand and climate change challenges.

Prentice; Rosemary Green;Abstract Facilitating dietary change is pivotal to improving population health, increasing food system resilience, and minimizing adverse impacts on the environment, but assessment of the current ‘status-quo’ and identification of bottlenecks for improvement has been lacking to date. We assessed deviation of the Gambian diet from the EAT-Lancet guidelines for healthy and sustainable diets and identified leverage points to improve nutritional and planetary health. We analysed the 2015/16 Gambian Integrated Household Survey dataset comprising food consumption data from 12 713 households. Consumption of different food groups was compared against the EAT-Lancet reference diet targets to assess deviation from the guidelines. We computed a ‘sustainable and healthy diet index (SHDI)’ based on deviation of different food groups from the EAT-Lancet recommendations and modelled the socio-economic and geographic determinants of households that achieved higher scores on this index, using multivariable mixed effects regression. The average Gambian diet had very low adherence to EAT-Lancet recommendations. The diet was dominated by refined grains and added sugars which exceeded the recommendations. SHDI scores for nutritionally important food groups such as fruits, vegetables, nuts, dairy, poultry, and beef and lamb were low. Household characteristics associated with higher SHDI scores included: being a female-headed household, having a relatively small household size, having a schooled head of the household, having a high wealth index, and residing in an urban settlement. Furthermore, diets reported in the dry season and households with high crop production diversity showed increased adherence to the targets. While average Gambian diets include lower amounts of food groups with harmful environmental footprint, they are also inadequate in healthy food groups and are high in sugar. There are opportunities to improve diets without increasing their environmental footprint by focusing on the substitution of refined grains by wholegrains, reducing sugar and increasing fruit and vegetables consumption.

However, for many countries, the optimal composition of locally contextualized healthy and sustainable diets is unknown. The Gambia is vulnerable to climate-change-induced future water scarcity which may affect crop yields and the ability to supply healthy diets. This study identifies potential shifts in Gambian diets that could make diets healthier and reduce the associated agricultural water footprint (WF), and assesses the cost and affordability implications of such dietary changes. Gambian Integrated Household Survey (IHS) food consumption data was combined with market prices, food expenditure and agricultural WF data. Current dietary patterns were compared with World Health Organization (WHO) dietary guidelines and optimized using linear programming to identify least-cost diets that met nutrition recommendations and reduced agricultural water use. Optimization scenarios explored the maximum reduction in green water use that could be achieved with ‘culturally-acceptable’ dietary shifts, and the magnitude of shifts required to maintain green water use at current levels. On average, current diets provide adequate energy and have appropriate macronutrient composition. However, only 14% of households consume enough fruit and vegetables (F&Vs), and consumption of added sugars exceeds recommendations. With ‘culturally-acceptable’ changes in consumption, agricultural water use could decrease by 10%–13% or increase by 9%, depending on the baseline dietary pattern. Extreme dietary shifts will be required to maintain water use at 2015 levels with projected population growth. To meet WHO recommendations, dietary costs would increase by 43% compared to the current baseline. Healthy and green water-saving diets would require 48%–63% of average household expenditure to purchase, which is unaffordable for almost half of the population. F&Vs alone account for 31%–40% of the cost of optimized diets compared to 12% of current diets. Dietary modification has the potential to improve the nutritional quality of Gambian diets while reducing agricultural water use, but the required changes are likely to be unaffordable for a large proportion of the population. Improving availability and affordability of nutritious foods—particularly F&Vs—will be crucial for the accessibility of healthy and sustainable diets in the Gambian population.

Dangour;Significance Environmental changes, including climate change, air pollution, and water scarcity and salinization, threaten global agricultural production, food security, and health. There is evidence that environmental change will reduce the yields of starchy staple crops, but impacts on (nonstaple) vegetables and legumes—important constituents of healthy diets—remain largely unknown. We systematically reviewed the available published evidence from experimental studies on the impact of environmental changes on yields and nutritional quality of (nonstaple) vegetables and legumes and found that environmental change would have a negative impact on yields without suitable responses from the agricultural sector. An enhanced understanding of the scale of environmental impacts on agricultural production is essential for the development of effective strategies to protect global population health.

Reducing food system emissions is an essential component of climate change mitigation, and a system-wide approach, including production, processing, trade and demand-side transformations, will be needed. Long-term analysis of greenhouse gas (GHG) emissions of food supply is crucial for informing this transformation, and understanding the processes contributing to existing trends can reveal opportunities for future mitigation strategies. To address these needs we used data on food supply, trade and emission intensity to quantify changes in GHG emissions between 1986 and 2017 resulting from food supply in the United Kingdom (UK). Uniquely, the relative contributions of supply-side and demand-side changes to historical trends in food emissions were assessed, and the gap between current UK food consumption and EAT-Lancet recommended diets was used to estimate the additional GHG emission reductions that could be achieved by shifting to the Planetary Health Diet (PHD). It was estimated that in the UK, per-capita GHG emissions from food fell by 32% (from 4.6 tCO2eq/capita to 3.1 tCO2eq/capita) between 1986 and 2017. Of this 32% reduction, 21% was due to supply-side changes (a fall in emission intensity per unit of production due to increased efficiency of farming practices), 10% was due to demand-side changes (including dietary change and waste reduction), and 2% was due to changing trade patterns. Relative to the PHD, however, the average UK citizen still greatly over-consumes beef, lamb and pork, tubers and starchy vegetables and dairy products, and under-consumes vegetables, nuts, and legumes. It was estimated that by adopting the PHD, UK per capita food emissions could be reduced by a further 42% to 1.8 tCO2eq/capita.
