Ethanol ablation (EA) of thyroid cystic nodules has been performed since the 1990s, but there is no global consensus or guideline. Although various limitations of EA have been described, recommendations for practical application are necessary. Therefore, the Task Force Committee of the Korean Society of Thyroid Radiology initiated the present consensus statement and here we provide recommendations for the role of EA in the management of symptomatic thyroid nodules. These recommendations are based on evidence to date from the literature and expert opinion.

In normal subjects, alcohol increases handwriting size, but the mechanism is not understood. Here we show that the alcohol effect on handwriting can be explained by a selective impairment of kinaesthetic perception. Thirty volunteers (15 male, aged 18-29 years) took part in an open study. They were tested before and after a drink containing vodka intended to produce a blood alcohol concentration of about 80mg/100ml. Tests included kinaesthetic distance estimation, in which volunteers worked with preferred hand and arm behind a screen which hid their movements; visual distance estimation; and measures of handwriting and drawing. Blood alcohol concentration at 55min, based on breathalyser measurements, was 76.7mg/100ml (SD 9.8). When asked to move the hand and mark a distance of 10cm from a starting point, distances estimates increased by 7-10% ( p 0.01). Similar increases were seen for writing words and drawing characters. Signatures were increased in height but not in length. Distances estimated visually were increased much less, by 3-4% ( p 0.05). Tests of psychomotor performance indicated the expected effects of ethanol. These results suggest that ethanol affects writing size by reducing kinaesthetically perceived distances.

The aim of this study was to test the hypothesis that the motion of the soft tissue of the lower leg contributes significantly to the attenuation of the forces during heel impacts. To examine this, a two-dimensional model of the shank and heel pad was developed using DADS. The model contained a heel pad element and a rigid skeleton to which was connected soft tissue which could move relative to the bone. Simulations permitted estimation of heel pad properties directly from heel pad deformations, and from the kinematics of an impacting pendulum. These two approaches paralleled those used in vitro and in vivo, respectively. Measurements from the pendulum indicated that heel pad properties changed from those found in vitro to those found in vivo as relative motion of the bone and soft tissue was allowed. This would indicate that pendulum measures of the in vivo heel pad properties are also measuring the properties of the whole lower leg. The ability of the wobbling mass of the shank to dissipate energy during an impact was found to be significant. These results demonstrate the important role of both the heel pad and soft tissue of the shank to the dissipation of mechanical energy during impacts. These results provide a further clarification of the paradox between the measurements of heel pad properties made in vivo and in vitro.

Self-poisoning in children is a serious health concern accounting for 2% and 5% of childhood deaths in the developed and developing world, respectively. Type of poison and intent varies between age groups, with alcohol poisoning becoming increasingly common in teenagers. The aim of the study was to identify the characteristics of paediatric self-poisoning at Ninewells Hospital, Dundee, as a function of age, sex, intent and type of poison. Data from patients under the age of 17 presenting to Ninewells Hospital's Emergency Department with self-poisoning from 1 December 2008 to 30 November 2009 inclusive were identified, classified and analysed using chi-square testing. Overall there was no significant difference in gender. However, females significantly dominated in the ≥12<17 years age group, with older females also significantly more likely to deliberately self-poison. Alcohol was the sole cause of accidental self-poisoning in this age group while paracetamol was used in the majority of cases of deliberate self-poisoning. In the <6 years age group, household chemical ingestion and over-the-counter medications were the most common poisons. The findings reflect previous published data and national trends. The prevalence of alcohol abuse in the ≥12<17 years age group is a major public health issue that must be addressed.

Spatiotemporal monitoring of urban CO2 emissions is crucial for developing strategies and actions to mitigate climate change. However, most spatiotemporal inventories do not adopt urban form data and have a coarse resolution of over 1 km, which limits their implications in intra-city planning. This study aims to model the spatiotemporal carbon emissions of the two largest mega-urban regions in China, the Yangtze River Delta and the Pearl River Delta, using urban form data from the Local Climate Zone scheme and landscape metrics, nighttime light images, and a year-fixed effects model at a fine resolution from 2012 to 2016. The panel data model has an R2 value of 0.98. This study identifies an overall fall in carbon emissions in both regions since 2012 and a slight elevation of emissions from 2015 to 2016. In addition, urban compaction and integrated natural landscapes are found to be related to low emissions, whereas scattered low-rise buildings are associated with rising carbon emissions. Furthermore, this study more accurately extracts urban areas and can more clearly identify intra-urban variations in carbon emissions than other datasets. The open data supported methodology, regression models, and results can provide accurate and quantifiable evidence at the community level for achieving a carbon-neutral built environment.

Alcohol-related trauma remains high among underserved patients despite ongoing preventive measures. Geographic variability in prevalence of alcohol-related injury has prompted reexamination of this burden across different regions. We sought to elucidate demographic and socioeconomic factors influencing the prevalence of alcohol-related trauma among underserved patients and determine alcohol effects on selected outcomes.A retrospective analysis examined whether patients admitted to a suburban trauma center differed according to their blood alcohol concentration (BAC) on admission. Patients were stratified based on their BAC into four categories (undetectable BAC, BAC 1-99mg/dL, BAC 100-199mg/dL, and BAC ≥ 200mg/dL). T-tests and X2 tests were used to detect differences between BAC categories in terms of patient demographics and clinical outcomes. Multivariate linear and logistic regressions were used to investigate the association between patient variables and selected outcomes while controlling for confounders.One third of 738 patients analyzed were BAC-positive, mean (SD) BAC was 211.4 (118.9) mg/dL, 80% of BAC-positive patients had levels ≥ 100mg/dL. After risk adjustments, the following patient characteristics were predictive of having highly elevated BAC (≥200mg/dL) upon admission to the Trauma Center; Hispanic patients (adjusted odds ratio (OR)=1.91, 95% confidence interval (CI): 1.14-3.21), unemployment (OR=1.74, 95% CI: 1.09-2.78), Medicaid beneficiaries (OR=3.59, 95% CI: 1.96-6.59), and uninsured patients (OR=2.86, 95% CI: 1.60-5.13). Patients with BAC of 100-199mg/dL were likely to be more severely injured (P=0.016) compared to undetectable-BAC patients. There was no association between being intoxicated, and being ICU-admitted or having differences in length of ICU or hospital stay.Demographic and socioeconomic factors underlie disparities in the prevalence of alcohol-related trauma among underserved patients. These findings may guide targeted interventions toward specific populations to help reduce the burden of alcohol-related injury.

Ethanol ablation (EA) of thyroid cystic nodules has been performed since the 1990s, but there is no global consensus or guideline. Although various limitations of EA have been described, recommendations for practical application are necessary. Therefore, the Task Force Committee of the Korean Society of Thyroid Radiology initiated the present consensus statement and here we provide recommendations for the role of EA in the management of symptomatic thyroid nodules. These recommendations are based on evidence to date from the literature and expert opinion.

In normal subjects, alcohol increases handwriting size, but the mechanism is not understood. Here we show that the alcohol effect on handwriting can be explained by a selective impairment of kinaesthetic perception. Thirty volunteers (15 male, aged 18-29 years) took part in an open study. They were tested before and after a drink containing vodka intended to produce a blood alcohol concentration of about 80mg/100ml. Tests included kinaesthetic distance estimation, in which volunteers worked with preferred hand and arm behind a screen which hid their movements; visual distance estimation; and measures of handwriting and drawing. Blood alcohol concentration at 55min, based on breathalyser measurements, was 76.7mg/100ml (SD 9.8). When asked to move the hand and mark a distance of 10cm from a starting point, distances estimates increased by 7-10% ( p 0.01). Similar increases were seen for writing words and drawing characters. Signatures were increased in height but not in length. Distances estimated visually were increased much less, by 3-4% ( p 0.05). Tests of psychomotor performance indicated the expected effects of ethanol. These results suggest that ethanol affects writing size by reducing kinaesthetically perceived distances.

The aim of this study was to test the hypothesis that the motion of the soft tissue of the lower leg contributes significantly to the attenuation of the forces during heel impacts. To examine this, a two-dimensional model of the shank and heel pad was developed using DADS. The model contained a heel pad element and a rigid skeleton to which was connected soft tissue which could move relative to the bone. Simulations permitted estimation of heel pad properties directly from heel pad deformations, and from the kinematics of an impacting pendulum. These two approaches paralleled those used in vitro and in vivo, respectively. Measurements from the pendulum indicated that heel pad properties changed from those found in vitro to those found in vivo as relative motion of the bone and soft tissue was allowed. This would indicate that pendulum measures of the in vivo heel pad properties are also measuring the properties of the whole lower leg. The ability of the wobbling mass of the shank to dissipate energy during an impact was found to be significant. These results demonstrate the important role of both the heel pad and soft tissue of the shank to the dissipation of mechanical energy during impacts. These results provide a further clarification of the paradox between the measurements of heel pad properties made in vivo and in vitro.

Self-poisoning in children is a serious health concern accounting for 2% and 5% of childhood deaths in the developed and developing world, respectively. Type of poison and intent varies between age groups, with alcohol poisoning becoming increasingly common in teenagers. The aim of the study was to identify the characteristics of paediatric self-poisoning at Ninewells Hospital, Dundee, as a function of age, sex, intent and type of poison. Data from patients under the age of 17 presenting to Ninewells Hospital's Emergency Department with self-poisoning from 1 December 2008 to 30 November 2009 inclusive were identified, classified and analysed using chi-square testing. Overall there was no significant difference in gender. However, females significantly dominated in the ≥12<17 years age group, with older females also significantly more likely to deliberately self-poison. Alcohol was the sole cause of accidental self-poisoning in this age group while paracetamol was used in the majority of cases of deliberate self-poisoning. In the <6 years age group, household chemical ingestion and over-the-counter medications were the most common poisons. The findings reflect previous published data and national trends. The prevalence of alcohol abuse in the ≥12<17 years age group is a major public health issue that must be addressed.

Spatiotemporal monitoring of urban CO2 emissions is crucial for developing strategies and actions to mitigate climate change. However, most spatiotemporal inventories do not adopt urban form data and have a coarse resolution of over 1 km, which limits their implications in intra-city planning. This study aims to model the spatiotemporal carbon emissions of the two largest mega-urban regions in China, the Yangtze River Delta and the Pearl River Delta, using urban form data from the Local Climate Zone scheme and landscape metrics, nighttime light images, and a year-fixed effects model at a fine resolution from 2012 to 2016. The panel data model has an R2 value of 0.98. This study identifies an overall fall in carbon emissions in both regions since 2012 and a slight elevation of emissions from 2015 to 2016. In addition, urban compaction and integrated natural landscapes are found to be related to low emissions, whereas scattered low-rise buildings are associated with rising carbon emissions. Furthermore, this study more accurately extracts urban areas and can more clearly identify intra-urban variations in carbon emissions than other datasets. The open data supported methodology, regression models, and results can provide accurate and quantifiable evidence at the community level for achieving a carbon-neutral built environment.

Alcohol-related trauma remains high among underserved patients despite ongoing preventive measures. Geographic variability in prevalence of alcohol-related injury has prompted reexamination of this burden across different regions. We sought to elucidate demographic and socioeconomic factors influencing the prevalence of alcohol-related trauma among underserved patients and determine alcohol effects on selected outcomes.A retrospective analysis examined whether patients admitted to a suburban trauma center differed according to their blood alcohol concentration (BAC) on admission. Patients were stratified based on their BAC into four categories (undetectable BAC, BAC 1-99mg/dL, BAC 100-199mg/dL, and BAC ≥ 200mg/dL). T-tests and X2 tests were used to detect differences between BAC categories in terms of patient demographics and clinical outcomes. Multivariate linear and logistic regressions were used to investigate the association between patient variables and selected outcomes while controlling for confounders.One third of 738 patients analyzed were BAC-positive, mean (SD) BAC was 211.4 (118.9) mg/dL, 80% of BAC-positive patients had levels ≥ 100mg/dL. After risk adjustments, the following patient characteristics were predictive of having highly elevated BAC (≥200mg/dL) upon admission to the Trauma Center; Hispanic patients (adjusted odds ratio (OR)=1.91, 95% confidence interval (CI): 1.14-3.21), unemployment (OR=1.74, 95% CI: 1.09-2.78), Medicaid beneficiaries (OR=3.59, 95% CI: 1.96-6.59), and uninsured patients (OR=2.86, 95% CI: 1.60-5.13). Patients with BAC of 100-199mg/dL were likely to be more severely injured (P=0.016) compared to undetectable-BAC patients. There was no association between being intoxicated, and being ICU-admitted or having differences in length of ICU or hospital stay.Demographic and socioeconomic factors underlie disparities in the prevalence of alcohol-related trauma among underserved patients. These findings may guide targeted interventions toward specific populations to help reduce the burden of alcohol-related injury.
