Local, regional, and global food systems are unable to provide consumers with nutritious and affordable diets. Suboptimal diets exacerbate the triple burden of malnutrition, with micronutrient deficiencies affecting more than two billion people, two billion people suffering from overweight, and more than 140 million children who are stunted. The unaffordability of nutritious diets represents an obstacle for many, especially in low- and middle-income countries where healthy diets are five times more expensive than starchy staple diets. Food system transformations are urgently required to provide consumers with more affordable and nutritious diets that are capable of meeting social and environmental challenges. In this review, we underline the critical role of innovation within the food system transformation discourse. We aim to define principles for implementing evidence-based and long-term food system innovations that are economically, socially, and environmentally sustainable and, above all, aimed at improving diets and public health. We begin by defining and describing the role of innovation in the transformation of food systems and uncover the major barriers to implementing these innovations. Lastly, we explore case studies that demonstrate successful innovations for healthier diets.

AbstractThis assessment by the Environmental Effects Assessment Panel (EEAP) of the United Nations Environment Programme (UNEP) provides the latest scientific update since our most recent comprehensive assessment (Photochemical and Photobiological Sciences, 2019, 18, 595–828). The interactive effects between the stratospheric ozone layer, solar ultraviolet (UV) radiation, and climate change are presented within the framework of the Montreal Protocol and the United Nations Sustainable Development Goals. We address how these global environmental changes affect the atmosphere and air quality; human health; terrestrial and aquatic ecosystems; biogeochemical cycles; and materials used in outdoor construction, solar energy technologies, and fabrics. In many cases, there is a growing influence from changes in seasonality and extreme events due to climate change. Additionally, we assess the transmission and environmental effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is responsible for the COVID-19 pandemic, in the context of linkages with solar UV radiation and the Montreal Protocol.

Since the onset of the COVID-19 crisis many have opinionated on how it may affect society’s response to climate change. Two key questions here are how COVID-19 is expected to influence climate action by citizens and by the government. We answer these by applying topic modelling to textual responses from a survey of Spanish citizens. The identified topics tend to be more negative than positive, and more optimistic concerning future climate action by citizens. Positive views involve increasing pro-environmental behavior and are more common among younger, higher educated and male respondents as well as among those who perceive climate change as a serious threat or positively assessed COVID-19 confinement. Negative topics express concern that financial resources for climate action will be limited due to a focus on healthcare and economic recovery. In addition, they mention government mismanagement and waste due to use of protective measures like masks and gloves as impediments to effective climate action.

Since the onset of the COVID-19 crisis many have opinionated on how it may affect society’s response to climate change. Two key questions here are how COVID-19 is expected to influence climate action by citizens and by the government. We answer these by applying topic modelling to textual responses from a survey of Spanish citizens. The identified topics tend to be more negative than positive, and more optimistic concerning future climate action by citizens. Positive views involve increasing pro-environmental behavior and are more common among younger, higher educated and male respondents as well as among those who perceive climate change as a serious threat or positively assessed COVID-19 confinement. Negative topics express concern that financial resources for climate action will be limited due to a focus on healthcare and economic recovery. In addition, they mention government mismanagement and waste due to use of protective measures like masks and gloves as impediments to effective climate action.

In particular, the current pandemic of Covid-19 has challenged the resilience of health systems. In many healthcare systems, primary care practices play a crucial role in the management of crises as they are often the first point of contact and main health care provider for patients. Therefore, this study explored which situations are perceived as crises by primary care practice teams and potential strategies for crisis management.MethodsA qualitative observational study was conducted. Data were collected in interviews and focus groups with experts from primary care practices and stakeholders focusing on primary care practices in Germany such as physicians, medical assistants, practice managers, quality managers, hygiene managers and institutions on health system level (politics, research and health insurance). All interviews and focus groups were audio-recorded and transcribed verbatim. A qualitative content analysis was performed using a rapid qualitative analysis approach first, followed by a thematic analysis.ResultsTwo focus groups and 26 interviews including 40 participating experts were conducted. Many different situations were perceived as crises, varying from issues in the practice organization to problems on health system level and international disasters. Distinct aspects associated with the perception of a crisis situation by interviewees were the presence of emotional reactions, a need for organizational changes and a lack of necessary resources. A broad spectrum of possible strategies was discussed that could help to cope with or even prevent the emergence of an actual crisis. In particular, strengthening communication within practice teams and resilience among employees was perceived to be fundamental for improving responses to crises or preventing them.ConclusionsThe study provides perspectives of primary health care workers on crises in health, that could inform health policy regarding prevention and management of future crises in primary care facilities.

The COVID-19 pandemic amplified the role of policy capacity in mounting this kind of integrated policy response; however, the relationship between pre- and post-pandemic SDG efforts remains largely unexplored. In this article, we seek to address this gap through a conceptual analysis of policy integration and the capacities necessary for its application to the current SDG situation. Building on the literature on policy design, we define policy integration as the process of effectively reconciling policy goals and policy instruments and we offer a typology of policy integration efforts based on the degree of goal and instrument consistency including: policy harmonization, mainstreaming, coordination, and institutionalization. These forms of policy integration dictate the types of strategies that governments need to adopt in order to arrive at a more coherent policy mix. Following the dimensions of policy capacity by Wu et al. (2015), policy capacities are identified that are critical to ensuring successful integration. This information, thus, contributes to both academic- and policy-related debates on policy integration, by advancing conceptual clarity on the different, and sometimes, diverging concepts used in the field.

The current COVID-19 pandemic has resulted in an immense and unforeseen increase in demand for personal protective equipment (PPE) for healthcare workers worldwide. Amongst other products, respirator masks are crucial to protect the users against transmission of the virus. Decontamination and reuse of the existing stock could be a solution to the shortage of new respirators. Based upon existing studies, it was found that (I) a solid quality control method is essential to test product reuse, (II) in-depth evaluation of the different parts of the filtering facepiece respirator (FFR) should be considered, and (III) communication of the reuse cycle is essential to take track of the amount of reuse, as this is limited to ensure quality. The goal of this paper is two-fold. First, we identify the impact of decontamination on the different parts of the FFRs and how the quality control should be performed. Two different types of FFRs are analysed within this paper, resulting in the recommendation of combining quantitative respirator mask fit testing with a thorough sensory evaluation of decontaminated FFRs to qualify them for reuse. Secondly, the possibilities of communication of this reuse to the eventual user are mapped through in-depth reasoning.

This paper provides a quantitative assessment of the economic and environmental impacts of these measures in the short and long term. We use a Computable General Equilibrium model designed to assess environmental and energy policies impacts at the macroeconomic and sectoral levels. We find that the lockdown has led to a significant decrease in economic output of 5% of GDP, but a positive environmental impact with a 6.6% reduction in CO2 emissions in 2020. Both decreases are temporary: economic and environmental indicators return to their baseline trajectory after a few years. CO2 emissions even end up significantly higher after the COVID-19 crisis when we account for persistently low oil prices. We then investigate whether implementing carbon pricing can still yield positive macroeconomic dividends in the post-COVID recovery. We find that implementing ambitious carbon pricing speeds up economic recovery while significantly reducing CO2 emissions. By maintaining high fossil fuel prices, carbon taxation reduces the imports of fossil energy and stimulates energy efficiency investments while the full redistribution of tax proceeds does not hamper the recovery.

It remains unclear how COVID-19 has affected public engagement with the climate crisis. According to the finite-pool-of-worry hypothesis, concern about climate change should have decreased after the pandemic, in turn reducing climate-policy acceptance. Here we test these and several other conjectures by using survey data from 1172 Spanish participants who responded before and after the first wave of COVID-19, allowing for both aggregate and within-person analyses. We find that on average climate concern has decreased, while acceptance of most climate policies has increased. At the individual-level, adverse health experiences are unrelated to these changes. The same holds for negative economic experiences, with the exception that unemployment is associated with reduced acceptance of some policies. Complementary to the finite-pool-of-worry test, we examine three additional pandemic-related issues. As we find, (1) higher climate concern and policy acceptance are associated with a belief that climate change contributed to the COVID-19 outbreak; (2) higher policy acceptance is associated with a positive opinion about how the government addressed the COVID-19 crisis; (3) citizens show favorable attitudes to a carbon tax with revenues used to compensate COVID-19-related expenditures. Overall, we conclude there is support for addressing the global climate crisis even during a global health crisis.

In the course of the COVID-19 pandemic, it has become clear that primary healthcare systems play a critical role in clinical care, such as patient screening, triage, physical and psychological support and also in promoting good community advice and awareness in coordination with secondary healthcare and preventive care. Because of the role of social and environmental factors in COVID-19 transmission and burden of disease, it is essential to ensure that there is adequate coordination of population-based health services and public health interventions. The COVID-19 pandemic has shown the primary and community healthcare (P&CHC) system’s weaknesses worldwide. In many instances, P&CHC played only a minor role, the emphasis being on hospital and intensive care beds. This was compounded by political failures, in supporting local community resilience. Placing community building, social cohesion and resilience at the forefront of dealing with the COVID-19 crisis can help align solutions that provide a vision of ‘planetary health’. This can be achieved by involving local well-being and participation in the face of any pervasive health and environmental crisis, including other epidemics and large-scale ecological crises. This paper proposes that P&CHC should take on two critical roles: first, to support local problem-solving efforts and to serve as a partner in innovative approaches to safeguarding community well-being; and second, to understand the local environment and health risks in the context of the global health perspective. We see this as an opportunity of immediate value and broad consequence beyond the control of the COVID-19 pandemic.

Especially, the energy sector is in no exception, with concerns being raised about how governments and private sector will recover from the economic recession but continue working towards clean energy transition. Firm commitments and targeted energy efficiency (EE) investments are required. This study's aim is twofold; firstly, a review on the energy sector crisis with the critical role of EE investments and the investigation of available Decision Support (DS) tools to boost such recovery. The methodology followed by a review on strategically selected countries to capture their energy recovery needs and strategies. Results showed that EE investments will assist in countering pandemic recession, therefore, innovative DS tools and standardisation methods are required. However, hardly any tool exists identifying sustainable investments with strong potential to meet their commitments. The Triple-A Horizon 2020 standardised tools are considered to play critical role enabling EE projects to get financed towards assessing the EE investments' risks.

Local, regional, and global food systems are unable to provide consumers with nutritious and affordable diets. Suboptimal diets exacerbate the triple burden of malnutrition, with micronutrient deficiencies affecting more than two billion people, two billion people suffering from overweight, and more than 140 million children who are stunted. The unaffordability of nutritious diets represents an obstacle for many, especially in low- and middle-income countries where healthy diets are five times more expensive than starchy staple diets. Food system transformations are urgently required to provide consumers with more affordable and nutritious diets that are capable of meeting social and environmental challenges. In this review, we underline the critical role of innovation within the food system transformation discourse. We aim to define principles for implementing evidence-based and long-term food system innovations that are economically, socially, and environmentally sustainable and, above all, aimed at improving diets and public health. We begin by defining and describing the role of innovation in the transformation of food systems and uncover the major barriers to implementing these innovations. Lastly, we explore case studies that demonstrate successful innovations for healthier diets.

AbstractThis assessment by the Environmental Effects Assessment Panel (EEAP) of the United Nations Environment Programme (UNEP) provides the latest scientific update since our most recent comprehensive assessment (Photochemical and Photobiological Sciences, 2019, 18, 595–828). The interactive effects between the stratospheric ozone layer, solar ultraviolet (UV) radiation, and climate change are presented within the framework of the Montreal Protocol and the United Nations Sustainable Development Goals. We address how these global environmental changes affect the atmosphere and air quality; human health; terrestrial and aquatic ecosystems; biogeochemical cycles; and materials used in outdoor construction, solar energy technologies, and fabrics. In many cases, there is a growing influence from changes in seasonality and extreme events due to climate change. Additionally, we assess the transmission and environmental effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is responsible for the COVID-19 pandemic, in the context of linkages with solar UV radiation and the Montreal Protocol.

AbstractThis assessment by the Environmental Effects Assessment Panel (EEAP) of the United Nations Environment Programme (UNEP) provides the latest scientific update since our most recent comprehensive assessment (Photochemical and Photobiological Sciences, 2019, 18, 595–828). The interactive effects between the stratospheric ozone layer, solar ultraviolet (UV) radiation, and climate change are presented within the framework of the Montreal Protocol and the United Nations Sustainable Development Goals. We address how these global environmental changes affect the atmosphere and air quality; human health; terrestrial and aquatic ecosystems; biogeochemical cycles; and materials used in outdoor construction, solar energy technologies, and fabrics. In many cases, there is a growing influence from changes in seasonality and extreme events due to climate change. Additionally, we assess the transmission and environmental effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which is responsible for the COVID-19 pandemic, in the context of linkages with solar UV radiation and the Montreal Protocol.

Since the onset of the COVID-19 crisis many have opinionated on how it may affect society’s response to climate change. Two key questions here are how COVID-19 is expected to influence climate action by citizens and by the government. We answer these by applying topic modelling to textual responses from a survey of Spanish citizens. The identified topics tend to be more negative than positive, and more optimistic concerning future climate action by citizens. Positive views involve increasing pro-environmental behavior and are more common among younger, higher educated and male respondents as well as among those who perceive climate change as a serious threat or positively assessed COVID-19 confinement. Negative topics express concern that financial resources for climate action will be limited due to a focus on healthcare and economic recovery. In addition, they mention government mismanagement and waste due to use of protective measures like masks and gloves as impediments to effective climate action.

In particular, the current pandemic of Covid-19 has challenged the resilience of health systems. In many healthcare systems, primary care practices play a crucial role in the management of crises as they are often the first point of contact and main health care provider for patients. Therefore, this study explored which situations are perceived as crises by primary care practice teams and potential strategies for crisis management.MethodsA qualitative observational study was conducted. Data were collected in interviews and focus groups with experts from primary care practices and stakeholders focusing on primary care practices in Germany such as physicians, medical assistants, practice managers, quality managers, hygiene managers and institutions on health system level (politics, research and health insurance). All interviews and focus groups were audio-recorded and transcribed verbatim. A qualitative content analysis was performed using a rapid qualitative analysis approach first, followed by a thematic analysis.ResultsTwo focus groups and 26 interviews including 40 participating experts were conducted. Many different situations were perceived as crises, varying from issues in the practice organization to problems on health system level and international disasters. Distinct aspects associated with the perception of a crisis situation by interviewees were the presence of emotional reactions, a need for organizational changes and a lack of necessary resources. A broad spectrum of possible strategies was discussed that could help to cope with or even prevent the emergence of an actual crisis. In particular, strengthening communication within practice teams and resilience among employees was perceived to be fundamental for improving responses to crises or preventing them.ConclusionsThe study provides perspectives of primary health care workers on crises in health, that could inform health policy regarding prevention and management of future crises in primary care facilities.

The COVID-19 pandemic amplified the role of policy capacity in mounting this kind of integrated policy response; however, the relationship between pre- and post-pandemic SDG efforts remains largely unexplored. In this article, we seek to address this gap through a conceptual analysis of policy integration and the capacities necessary for its application to the current SDG situation. Building on the literature on policy design, we define policy integration as the process of effectively reconciling policy goals and policy instruments and we offer a typology of policy integration efforts based on the degree of goal and instrument consistency including: policy harmonization, mainstreaming, coordination, and institutionalization. These forms of policy integration dictate the types of strategies that governments need to adopt in order to arrive at a more coherent policy mix. Following the dimensions of policy capacity by Wu et al. (2015), policy capacities are identified that are critical to ensuring successful integration. This information, thus, contributes to both academic- and policy-related debates on policy integration, by advancing conceptual clarity on the different, and sometimes, diverging concepts used in the field.

The current COVID-19 pandemic has resulted in an immense and unforeseen increase in demand for personal protective equipment (PPE) for healthcare workers worldwide. Amongst other products, respirator masks are crucial to protect the users against transmission of the virus. Decontamination and reuse of the existing stock could be a solution to the shortage of new respirators. Based upon existing studies, it was found that (I) a solid quality control method is essential to test product reuse, (II) in-depth evaluation of the different parts of the filtering facepiece respirator (FFR) should be considered, and (III) communication of the reuse cycle is essential to take track of the amount of reuse, as this is limited to ensure quality. The goal of this paper is two-fold. First, we identify the impact of decontamination on the different parts of the FFRs and how the quality control should be performed. Two different types of FFRs are analysed within this paper, resulting in the recommendation of combining quantitative respirator mask fit testing with a thorough sensory evaluation of decontaminated FFRs to qualify them for reuse. Secondly, the possibilities of communication of this reuse to the eventual user are mapped through in-depth reasoning.

The current COVID-19 pandemic has resulted in an immense and unforeseen increase in demand for personal protective equipment (PPE) for healthcare workers worldwide. Amongst other products, respirator masks are crucial to protect the users against transmission of the virus. Decontamination and reuse of the existing stock could be a solution to the shortage of new respirators. Based upon existing studies, it was found that (I) a solid quality control method is essential to test product reuse, (II) in-depth evaluation of the different parts of the filtering facepiece respirator (FFR) should be considered, and (III) communication of the reuse cycle is essential to take track of the amount of reuse, as this is limited to ensure quality. This paper provides a quantitative assessment of the economic and environmental impacts of these measures in the short and long term. We use a Computable General Equilibrium model designed to assess environmental and energy policies impacts at the macroeconomic and sectoral levels. We find that the lockdown has led to a significant decrease in economic output of 5% of GDP, but a positive environmental impact with a 6.6% reduction in CO2 emissions in 2020. Both decreases are temporary: economic and environmental indicators return to their baseline trajectory after a few years. CO2 emissions even end up significantly higher after the COVID-19 crisis when we account for persistently low oil prices. We then investigate whether implementing carbon pricing can still yield positive macroeconomic dividends in the post-COVID recovery. We find that implementing ambitious carbon pricing speeds up economic recovery while significantly reducing CO2 emissions. By maintaining high fossil fuel prices, carbon taxation reduces the imports of fossil energy and stimulates energy efficiency investments while the full redistribution of tax proceeds does not hamper the recovery.

It remains unclear how COVID-19 has affected public engagement with the climate crisis. According to the finite-pool-of-worry hypothesis, concern about climate change should have decreased after the pandemic, in turn reducing climate-policy acceptance. Here we test these and several other conjectures by using survey data from 1172 Spanish participants who responded before and after the first wave of COVID-19, allowing for both aggregate and within-person analyses. We find that on average climate concern has decreased, while acceptance of most climate policies has increased. At the individual-level, adverse health experiences are unrelated to these changes. The same holds for negative economic experiences, with the exception that unemployment is associated with reduced acceptance of some policies. Complementary to the finite-pool-of-worry test, we examine three additional pandemic-related issues. As we find, (1) higher climate concern and policy acceptance are associated with a belief that climate change contributed to the COVID-19 outbreak; (2) higher policy acceptance is associated with a positive opinion about how the government addressed the COVID-19 crisis; (3) citizens show favorable attitudes to a carbon tax with revenues used to compensate COVID-19-related expenditures. Overall, we conclude there is support for addressing the global climate crisis even during a global health crisis.

In the course of the COVID-19 pandemic, it has become clear that primary healthcare systems play a critical role in clinical care, such as patient screening, triage, physical and psychological support and also in promoting good community advice and awareness in coordination with secondary healthcare and preventive care. Because of the role of social and environmental factors in COVID-19 transmission and burden of disease, it is essential to ensure that there is adequate coordination of population-based health services and public health interventions. The COVID-19 pandemic has shown the primary and community healthcare (P&CHC) system’s weaknesses worldwide. In many instances, P&CHC played only a minor role, the emphasis being on hospital and intensive care beds. This was compounded by political failures, in supporting local community resilience. Placing community building, social cohesion and resilience at the forefront of dealing with the COVID-19 crisis can help align solutions that provide a vision of ‘planetary health’. This can be achieved by involving local well-being and participation in the face of any pervasive health and environmental crisis, including other epidemics and large-scale ecological crises. This paper proposes that P&CHC should take on two critical roles: first, to support local problem-solving efforts and to serve as a partner in innovative approaches to safeguarding community well-being; and second, to understand the local environment and health risks in the context of the global health perspective. We see this as an opportunity of immediate value and broad consequence beyond the control of the COVID-19 pandemic.

In the course of the COVID-19 pandemic, it has become clear that primary healthcare systems play a critical role in clinical care, such as patient screening, triage, physical and psychological support and also in promoting good community advice and awareness in coordination with secondary healthcare and preventive care. Because of the role of social and environmental factors in COVID-19 transmission and burden of disease, it is essential to ensure that there is adequate coordination of population-based health services and public health interventions. The COVID-19 pandemic has shown the primary and community healthcare (P&CHC) system’s weaknesses worldwide. In many instances, P&CHC played only a minor role, the emphasis being on hospital and intensive care beds. This was compounded by political failures, in supporting local community resilience. Placing community building, social cohesion and resilience at the forefront of dealing with the COVID-19 crisis can help align solutions that provide a vision of ‘planetary health’. This can be achieved by involving local well-being and participation in the face of any pervasive health and environmental crisis, including other epidemics and large-scale ecological crises. This paper proposes that P&CHC should take on two critical roles: first, to support local problem-solving efforts and to serve as a partner in innovative approaches to safeguarding community well-being; and second, to understand the local environment and health risks in the context of the global health perspective. We see this as an opportunity of immediate value and broad consequence beyond the control of the COVID-19 pandemic.

Especially, the energy sector is in no exception, with concerns being raised about how governments and private sector will recover from the economic recession but continue working towards clean energy transition. Firm commitments and targeted energy efficiency (EE) investments are required. This study's aim is twofold; firstly, a review on the energy sector crisis with the critical role of EE investments and the investigation of available Decision Support (DS) tools to boost such recovery. The methodology followed by a review on strategically selected countries to capture their energy recovery needs and strategies. Results showed that EE investments will assist in countering pandemic recession, therefore, innovative DS tools and standardisation methods are required. However, hardly any tool exists identifying sustainable investments with strong potential to meet their commitments. The Triple-A Horizon 2020 standardised tools are considered to play critical role enabling EE projects to get financed towards assessing the EE investments' risks.
